Simple Tools Used in the management of Diabetic Neuropathy Peripheral neuropathy is the prime pathogenic ingredient in the recipe for diabetic foot ulceration. Ulceration, in turn, is the key factor that may precipitate a cascade of events leading to lower-extremity amputation. Clearly, identifying the presence of neuropathy (or more pragmatically, clinically significant loss of protective sensation) and taking action before any breach in the epithelium is of enormous import in any screening and treatment program designed to prevent amputation. Several modalities have been advocated to assist in this endeavour. The 10-g monofilament has become the most widely promoted of these tools. It has been touted as an inexpensive, practical, and easy-to use device to detect loss of protective sensation. It consists of a card handle connected to a nylon monofilament, which measures clinically significant large-fibre neuropathy by buckling at 10 gm of force. TIPTHERM is an early diagnostic device for symmetrical polyneuropathy which measures temperature sensitivity of the skin. TIPTHERM was developed to give the clinician and patient a convenient and easy to use method of testing. It works up to a temperature of 23° C . Failure to perceive variances in temperature in the extremities is the most decisive early symptom of distal symmetric polyneuropathy, a typical ailment accompanying diabetes mellitus (Ziegler 1988). Diabetic peripheral neuropathy, one of the most common chronic complications, may be asymptomatic in up to 50% of cases. All patients with diabetes should therefore undergo a careful clinical examination of their feet at least once a year. Current screening methods include the 10gm monofilament and the vibration perception threshold. Vibration perception threshold (VPT) is considered as a gold standard for diagnosis of diabetic peripheral neuropathy. VibraTip™ has been specifically designed to overcome the limitations of tuning forks for testing the integrity of vibration sense in clinic and by the bedside. With sufficient battery life for several months of routine use, VibraTip™ is easily wiped clean and small enough to be concealed in the hand. Its silent switch and quiet operation minimizes cueing and its rounded head allows vibration sense to be assessed specifically and rapidly from almost any angle. This disposable device works at 128Hz and produce a vibration strength equivalent to 25Volts and hence testing this device is easier. When the patient feels the vibration at the test site in the foot then they are considered normal and when they don’t feel the sense then they are considered at high risk.
